NHS Choices - GP Online Services

We currently offer online services to include a summary of your medical record, which gives a list of your current medications, and list of any allergies/adverse reactions that we have recorded. This is in addition to being able to book/cancel appointments and order repeat prescriptions.

If you wish to access these services please download and complete the form and return to the surgery with 2 forms of ID, (1x photo and 1x proof of address).

From the 1 March 2016 the practice will be offering patients the facility to view their detailed coded medical record. This will enable you to view, for example immunisations, problems/diagnoses, consultation and procedure codes. It will not include any free text, letters and attachments or administration items. Approval Process:

Applications for this service may take up to 14 days subject to approval by a GP.  However the surgery has the right to refuse an application based on the best interests of the patient.

 

Please bear in mind that when viewing the information that you can see online it may be misleading if you rely on it alone to complete insurance, employment or legal reports or forms.
